1. Complete all sections applicable to you carefully.
2. Write in BLOCK LETTERS in the spaces provided or tick in the appropriate box.
3. Supplying of false information is a criminal offence and will result in automatic disqualification and rejection of the application. Further action may be taken.
4. Attach two (2) certified copies of each academic certificate, BIRTH certificate and ID.
5. Submit the completed application form to the Registry Office, Hillside Teachers’ College or online through the college website.
